The Leukemia & Lymphoma Society, Inc. ("LLS") was created to cure leukemia, lymphoma, Hodgkin's disease and myeloma, and improve the quality of life of patients and their families. LLS is recognized as tax exempt pursuant to Section 501(c)(3) of the Internal Revenue Code (EIN: 13-5644916).
Information concerning LLS, including financial statements or charitable purposes, may be obtained, without cost, by writing to the following address: The Leukemia & Lymphoma Society Mail Center, Attn: Finance Department, 1201 15th St., N.W., Suite 410, Washington, D.C. 20005 or by calling 914-949-5213. In addition, residents of the following states may obtain financial and/or licensing information from their states, as indicated.
Florida: Registration No. CH672. A COPY OF THE OFFICIAL REGISTRATION AND FINANCIAL INFORMATION FOR LLS MAY BE OBTAINED FROM THE DIVISION OF CONSUMER SERVICES BY CALLING TOLL-FREE (800-435-7352) WITHIN THE STATE. REGISTRATION DOES NOT IMPLY ENDORSEMENT, APPROVAL OR RECOMMENDATION BY THE STATE.
Georgia: A full and fair description of the programs of LLS and our financial statement summary is available upon request at the office and phone number indicated above.
Kansas: The official registration and annual financial report of LLS for its preceding fiscal year is on file with the Kansas Attorney General. Kansas Registration # 21-022873.
Maryland: Documents and information submitted under the Maryland Charitable Solicitations Act are available for the cost of postage and copies from the Maryland Secretary of State, State House, Annapolis, MD 21401 (410-974-5534).
Michigan: MICS # 9646.
Mississippi: The official registration and financial information of LLS may be obtained from the Mississippi Secretary of State's office by calling 1-888-236-6167. Registration with the Secretary of State does not imply endorsement by the Secretary of State.
Nevada: Contributions may be tax deductible pursuant to the provisions of section 170(c) of the Internal Revenue Code of 1986, 26 U.S.C. § 170(c).
New Jersey: INFORMATION FILED WITH THE ATTORNEY GENERAL CONCERNING THIS CHARITABLE SOLICITATION AND THE PERCENTAGE OF CONTRIBUTIONS RECEIVED BY THE CHARITY DURING THE LAST REPORTING PERIOD THAT WERE DEDICATED TO THE CHARITABLE PURPOSE MAY BE OBTAINED FROM THE ATTORNEY GENERAL OF THE STATE OF NEW JERSEY BY CALLING 973-504-6215 AND IS AVAILABLE ON THE INTERNET AT www.state.nj.us/lps/ca2/charities/. REGISTRATION WITH THE ATTORNEY GENERAL DOES NOT IMPLY ENDORSEMENT.
New York: Upon request, a copy of LLS’s (00-75-25) most recent annual report filed with the Attorney General is available from the organization or from the New York Office of the Attorney General by visiting its website (www.charitiesnys.com) or writing to the Charities Bureau at 28 Liberty Street, New York, NY 10005. Information about charitable organizations is also available by calling the New York Office of the Attorney General at (212) 416-8401.
North Carolina: Financial information about LLS and a copy of its license are available from the State Solicitation Licensing Branch at 1-888-830-4989 (within North Carolina) or (919) 814-5400 (outside of North Carolina). The license is not an endorsement by the state.
Pennsylvania: The official registration and financial information of LLS may be obtained from the Pennsylvania Department of State, Charities Bureau, by calling toll-free within Pennsylvania 1-800-732-0999. Registration does not imply endorsement.
Virginia: A copy of the latest financial statement of LLS is available upon request from the Virginia Department of Agriculture and Consumer Services, at P.O. Box 526, Richmond, VA 23218-0526, or by phone at (804) 786-1343.
Washington: LLS is registered with the Washington Secretary of State as required by law and additional financial information is available from the Secretary of State online at www.sos.wa.gov/charities, or for Washington residents, calling toll-free to 1-800-332-4483, or on file at Charities Division, Office of the Secretary of State, State of Washington, Olympia, WA 98504.
West Virginia: West Virginia residents may obtain a summary of the registration and financial documents of LLS from the Secretary of State, State Capitol, Charleston, WV 25305. Registration does not imply endorsement.
Wisconsin: A financial statement disclosing the assets, liabilities, fund balances, revenue, and expenses for LLS’s last fiscal year will be provided to any person upon request.
